At the Corner House Dental Practice we are proud to provide a high level of dental care for all our patients. For us to maintain these standards and to ensure that your journey with us is pleasant, we have some terms and conditions that we would like to set out.
We ask for your email address and mobile number for our records. This will only be used to send you reminders for upcoming dental appointments and also to send out dental recall reminders. Occasionally, we also have special offers and promotions which we can inform you of via email. Your personal details are kept entirely confidential and are not shared with anyone.
At every check up or at your first visit with us, you will be asked to fill out or update your Medical History form. This is very important as we need to know of any medications or changes to your health as sometimes these can have implications on dental treatment.
We understand that life can get busy and things can occur which may cause you to miss your dental appointment. However, we require 24 hour notification for any cancellations to allow enough time to offer that space to another patient in need of the time. Failure to do so will mean the following:
NHS patients who fail two appointments within a year or repeatedly cancel appointments, will either be taken off our books or will only be seen privately. They will be required to pay prior to being seen to secure their appointments.
Private patients who fail to attend appointments or cancel appointments repeatedly will be asked to pay a broken appointment fee. Thereafter they will be required to pay in advance for any appointments to secure their time, or they may be asked to find another practice.
Failing to attend a Dental Hygienist appointment will incur a missed appointment fee.
